Tools Help Physicians Treat Diabetic Patients

The Pennsylvania Medical Society’s diabetes care recommendations and diabetes mellitus flow chart can help physicians treat the growing number of patients with Type 2 diabetes.  

These tools are more important than ever as the percentage of Pennsylvania patients with diabetes has swelled from 5.5 percent in 1994 to 8.7 percent in 2008, according to data from the Centers for Disease Control and Prevention.  That translates to nearly one in 11 Pennsylvanians having diabetes. 

Updated in May 2010, the tools also can help physician practices meet quality reporting requirements. 

The recommendations review treatment goals, screening, diagnosis, and assessment and management of the diabetic patient. The flow sheet gives recommended elements for every diabetic visit and annual exam and allows physicians to record data and other notes. 

Based on recommendations from the American Diabetes Association (ADA), the information is the result of meetings held through the Pennsylvania Medical Society’s Medical Directors’ Forum, a coalition of Society physician leadership and medical directors from Pennsylvania health plans.

Under ADA recommendations, the goal is an HbA1c level of <7.0% as indicated under the GOALS section of our care recommendations document and also on the flow sheet. In addition, according to ADA, an HbA1c > 6.5% should be considered when diagnosing as is indicated under the DIAGNOSIS section of the care recommendations document.

Appreciate diabetic care recs and flow sheet. You cite an A1C under 7.0%, certainly the ADA rec, but on the Flow Sheet you cite a target of A1C below 6.5%, which is more in keeping with what Endocrinologists recommend. What is it? Need consistency.
